Use the form below to refer an older adult who would enjoy receiving friendly, handwritten cards in the mail from volunteers in our Cards Connect program.
Please confirm the following before submitting.
Cards Connect Eligibility
The person is 62 or older and lives in Minnesota
You’ve confirmed they want to receive cards in the mail
You have their correct mailing address
If they move, you or the recipient will be responsible for providing the updated address
This is not a dementia care or emergency support program
